Appendix A–C
Below is the authorised register of all Court forms, referenced throughout the Civil Procedure Rules.
Each form must bear the Court crest, title header, and blue-ink autograph field.
Appendix A – Procedural Forms
Code Title Purpose
FPC-00 Pre-Action Notice of Intent Allowing a claimant to notify another party of intention to file under postal venue, giving 7–14 days to settle before FPC-01 is lodged.
FPC-01 Notice of Claim Initiates a formal proceeding and sets out the claimant’s cause and relief sought.
FPC-02 Certificate of Service Verifies and certifies lawful delivery of all pleadings or notices to relevant parties.
FPC-03 Response To Claim Provides the respondent’s reply, defence, or admissions to the filed claim.
FPC-04 Counter Claim Notice Lodges a cross-action or independent claim arising from the same facts in issue.
FPC-05 Notice Of Hearing or Direction Requests or acknowledges the scheduling of a postal, digital, or in-person hearing.
FPC-06 Schedule Of Exhibit Lists, indexes, and authenticates all documentary and material evidence submitted.
FPC-07 Evidence Authentication Certificate Affirms truth and authorship of attached evidence through oath or autograph.
FPC-08 Notice of Correction Formally amends or rectifies an earlier filing, clerical error, or procedural defect.
Appendix B – Certification & Enforcement
Code Title Purpose
FPC-09 Certificate of Award Certifies the adjudicated decision and quantum of the award issued by the Judicial Post Master.
FPC-10 Notice of Settlement Records completion of negotiation or satisfaction of claim through mutual agreement.
FPC-11 Notice of Default Confirms failure by a party to perform or respond within the prescribed period.
FPC-11A A Certificate of Non-Performance Issued by the Registrar after expiry of compliance deadline (linked to FPC-12 Performance Certificate). Confirms the respondent has failed to act or pay. Sits directly under Part 8 – Remedies, Enforcement & Settlement.
FPC-12 Performance Certificate Confirms fulfilment of obligations, compliance with order, or execution of award.
FPC-13 Request for Record Access of Copy Seeks official access to or duplication of certified court or postal-court records.
FPC-14 Fee Waver Application Requests exemption or reduction of prescribed filing or service fees.
FPC-15 Receipt and Payment Record Registers monetary receipts, disbursements, or satisfaction of award payments.
FPC-15A Security Bond / Escrow Registration Records any bond, lien, or performance security lodged with the Court pending fulfilment of an award.
FPC-15B Enforcement Liaison Record Logs transmission of a certified award to another recognised venue or tribunal for enforcement.
Appendix C – Administrative and Amendment Forms
Code Title Purpose
FPC-16 Amendment Notice Introduces or records an alteration to governing rules, charters, or procedures.
FPC-17 Interpretation Record Registers an official interpretive note or clarification of procedural meaning or clause intent.
FPC-17A Registrar’s Annual Report Year-end statistical and administrative summary; supports audit integrity and public transparency.
FPC-18 Application for Leave to Enforce Award Formal application used when escalating a defaulted award to a public court (High Court KB Division) under Section 66 of the Arbitration Act 1996.
FPC-18A Affidavit of Authenticity of Award Sworn declaration by Judicial Post Master confirming that the award was lawfully issued and sealed attached to FPC-18 filings.
FPC-18B Writ or Warrant Record Entry Internal log sheet to record the High Court writ number and execution result (back-entry into the FPC ledger).
FPC-19 Application for Review/Reconsideration. Allows a party to request limited review of a determination on grounds of procedural error, new evidence, or manifest mistake. Not an appeal. Ensures fairness and internal accountability.
FPC-20 Supplementary Evidence Filing Notice Permits a party to lodge further evidence after filing FPC-06, provided justification is supplied and accepted by the Registrar or Judicial Post Master.
FPC-21 Certificate of Final Closure Officially marks the conclusion of all proceedings, enforcement, settlement, or compliance. Closes the ledger entry and finalises the case status.
Appendix D – International & Reciprocal Forms
Below is the authorised register of all international and reciprocal Court forms.
Each form must bear the Court crest, title header, and Seal of Veritas watermark, and be signed in blue ink by the authorised Registrar or Judicial Post Master.
Code Title Purpose
FPC-INT-01 Foreign Notice of Claim Registers a claim filed by a foreign claimant or tribunal under postal-syntax jurisdiction for adjudication before FPC-UK.
FPC-INT-02 Acknowledgment of Jurisdiction Confirms mutual recognition and acceptance of lawful authority between FPC-UK and the foreign court or arbitration venue.
FPC-INT-03 Reciprocal Service Certificate Certifies lawful postal or courier service of documents exchanged between recognised international venues.
FPC-INT-04 International Award Registration Records or recognises determinations and awards issued abroad for enforcement under postal-contract law.
FPC-INT-05 Dual-Seal Authentication Certificate (RSAP) Verifies authenticity of both the FPC-UK seal and the foreign- venue seal through the Reciprocal Seal Authentication Protocol (RSAP) and records ledger entry.
FPC-INT-06 Bilingual / Mirror Translation Record Certifies that a foreign-language translation is a faithful mirror of the original syntax-certified instrument, ensuring cross-border validity.
